Because of the correction of several metabolic abnormalities like dysglycemia, dyslipidemia, hypertension and other aspects of metabolic syndrome, these surgeries are also termed as metabolic surgery. Malabsorptive bariatric surgeries score better than restrictive surgeries. Both foregut and hindgut theories, along with alteration of other factors like other gut hormones, bile acids, gut microbiota, do play a role in remission of diabetes. Worldwide, Roux-en-Y gastric bypass is the most commonly done procedure followed by others like sleeve gastrectomy, gastric banding, bilio-pancreatic diversion, etc. Remission of diabetes is seen in 50-75% cases. We have used another technique called ileal interposition with sleeve gastrectomy or its modified diverted version with remission in diabetes rates ranging from 65-80%.
